To help this call run smoothly, please have the following information available:

Personal identification details (Social Security number, driver’s license, email)
Basic medical information (primary care physician and current medications)
Owner information if different from the insured
Beneficiary details (birthdates, addresses, phone numbers)
Information on any existing life insurance policies
Preferred premium payment method (EFT recommended; bring a voided check if applicable)
